Introduction: Near-infrared spectroscopy (NIRS) provides continuous, noninvasive monitoring of regional cerebral oxygen saturation (rSO₂) and may detect impaired cerebral perfusion as a trend monitor. This case report describes the intraoperative detection and management of unilateral cerebral desaturation during prone cervical fixation in a patient with suspected vertebral artery dissection.
Case Presentation: A 50-year-old female patient with no known comorbidities underwent C4–5–6 internal fixation surgery due to a C5 vertebral body fracture following an in-vehicle traffic accident. Preoperative magnetic resonance angiography revealed a suspected dissection of the right vertebral artery at the V2–3 segment. In addition to standard monitoring, invasive arterial blood pressure and bilateral cerebral NIRS monitoring were applied. During surgery performed in the prone position under general anesthesia, a marked decrease in right-sided cerebral rSO₂ values compared to baseline was observed during the first hour of the procedure. After verification of neck position and NIRS probe placement, mean arterial pressure was increased, fraction of inspired oxygen (FiO₂) was raised, and mild hypercapnia was maintained. Following these interventions, cerebral rSO₂ values returned to baseline levels, and the surgery was completed without complications. The postoperative course was uneventful, and the patient was discharged without new neurological complications.
Conclusion: Cerebral NIRS monitoring may help detect intraoperative cerebral desaturation in high-risk cervical trauma patients, even when conventional hemodynamic parameters appear acceptable. It may serve as an adjunctive tool for individualized hemodynamic optimization, particularly when vertebral artery injury is suspected.
