Amaç: Penil fraktür, ereksiyon halindeki peniste tunika albugineanın yırtılması ile karakterize, acil cerrahi müdahale gerektiren nadir bir ürolojik acildir. Erken cerrahi onarım günümüzde altın standart tedavi yaklaşımı olarak kabul edilmekte olup, buna rağmen hastaların bir kısmında erektil disfonksiyon, penil deviasyon, ağrılı boşalma ve yara yeri enfeksiyonu gibi postoperatif komplikasyonlar gelişebilmektedir. Bu çalışmada, penil fraktür onarımı sonrası komplikasyon gelişimini bağımsız olarak öngören klinik ve cerrahi faktörlerin belirlenmesi amaçlandı.
Gereç ve Yöntem: 2013–2023 yılları arasında tek bir tersiyer merkezde penil fraktür nedeniyle cerrahi onarım uygulanan 55 hasta retrospektif olarak incelendi. Hastalara ait demografik veriler, başvuru süresi, komorbid hastalık varlığı, defekt lokalizasyonu, defekt büyüklüğü, üretral yaralanma varlığı ve postoperatif sonuçlar kaydedildi. Tüm hastalara subkoronal degloving yaklaşımı ile cerrahi onarım uygulandı. Komplikasyon gelişimi ile ilişkili faktörler tek değişkenli analizler ve çok değişkenli lojistik regresyon modelleri kullanılarak değerlendirildi.
Bulgular: Ortalama yaş 40,7±12,4 yıl, ortalama başvuru süresi 7,0±5,3 saatti. Postoperatif komplikasyon 12 hastada (%21,8) gelişti. Gelişen komplikasyonlar ağrılı boşalma (n=5), penil deviasyon (n=4), erektil disfonksiyon (n=1) ve yara yeri enfeksiyonu (n=2) olarak saptandı. Çok değişkenli analizde defekt büyüklüğü (OR=1,172, %95 GA: 1,001–1,372; p=0,049) ve komorbidite varlığı (p=0,008) komplikasyon gelişiminin bağımsız prediktörleri olarak bulundu. Regresyon modeli komplikasyonlardaki varyansın %57,9'unu açıkladı (Nagelkerke R²=0,579).
Sonuç: Geniş tunikal defekt büyüklüğü ve komorbid hastalık varlığı, penil fraktür onarımı sonrası komplikasyon gelişiminin bağımsız öngördürücüleridir. Bu özelliklere sahip hastalar postoperatif dönemde daha yakın izlenmeli ve komplikasyonlar açısından dikkatle değerlendirilmelidir. Bulguların doğrulanması için daha geniş örneklemli, prospektif ve çok merkezli çalışmalara ihtiyaç vardır.

Background: Penile fracture is a rare urological emergency characterized by a tear in the tunica albuginea of the erect penis and requiring urgent surgical intervention. Although early surgical repair is currently accepted as the gold standard treatment approach, some patients may develop postoperative complications such as erectile dysfunction, penile deviation, painful ejaculation, and wound site infection. The aim of this study was to identify clinical and surgical factors that independently predict the development of complications following penile fracture repair.
Methods: A retrospective analysis was conducted on 55 patients who underwent surgical repair for penile fracture at a single tertiary center between 2013 and 2023. Demographic data, time to presentation, presence of comorbidities, defect location, defect size, presence of urethral injury, and postoperative outcomes were recorded for all patients. All patients underwent surgical repair using the subcoronal degloving approach. Factors associated with the development of complications were evaluated using univariate analyses and multivariate logistic regression models.
Results: The mean age was 40.7 ± 12.4 years, and the mean time to presentation was 7.0 ± 5.3 hours. Postoperative complications developed in 12 patients (21.8%). The complications that developed were painful ejaculation (n=5), penile deviation (n=4), erectile dysfunction (n=1), and wound site infection (n=2). In the multivariate analysis, defect size (OR=1.172, 95% CI: 1.001–1.372; p=0.049) and the presence of comorbidities (p=0.008) were found to be independent predictors of complication development. The regression model explained 57.9% of the variance in complications (Nagelkerke R² = 0.579).
Conclusion: A large tunical defect size and the presence of comorbidities are independent predictors of complication development following penile fracture repair. Patients with these characteristics should be monitored more closely during the postoperative period and carefully evaluated for complications. Larger, prospective, and multicenter studies are needed to validate these findings.
